We will teach our child to know and to love God our Father, his Son Jesus and the Holy Spirit. We believe that as baptized persons we are members of the family of God. DOCUMENTATION REQUIRED for INFANT or SCHOOL-AGED BAPTISM

Please drop off or email the following documentation at least one month prior to the scheduled Baptism to the Administrative Assistant in the Parish Office (see Contact Us on this page):

1. A copy of your child’s birth certificate.

2. Parent/s: A copy of mom or dad’s Baptismal Certificate (either parent).

3. Godparent/s: A copy of Confirmation certificate or a copy annotated Baptismal certificate noting Confirmation date on the certificate.

4. *If legal guardian, present a copy of guardianship and signed permission from birth parent acknowledging request for baptism.
